How extreme failure‐data filtering leads to poor process safety

چکیده

This paper describes both appropriate and inappropriate practices of filtering field failure data (FFD) prior to using the resulting count from estimate rates. The underlying causes (extreme) FFD are examined, consequences (poor process safety) discussed. An example is presented illustrating benefits all real failures discovered in estimation rates based on FFD.
